Understanding Eyeglass Prisms: Solutions for Eye Misalignment and Double Vision

Prism lenses are incorporated into eyeglasses to address binocular vision issues—where the eyes do not work together as a team. Depending on the severity of the misalignment, a patient may need a small amount of prism or a higher, more significant prescription.

Prism works by redirecting light so that images from both eyes align properly into a single, cohesive view.

Mild vs. Severe Misalignment

• Mild Cases: Subtle misalignment often causes symptoms like visual fatigue, eye strain, or persistent headaches. Treatment options may include vision therapy, prescription prism lenses, or a combination of both.
• Severe Cases: Significant misalignment typically leads to double vision (diplopia), where images appear separated horizontally or vertically. In these situations, prism is essential to restore single vision.

• Post-Surgical Changes: Ocular surgeries can sometimes disrupt eye muscles or unmask a pre-existing, subtle misalignment. For instance, double vision can occasionally follow cataract surgery or more invasive procedures like a scleral buckle for retinal repair.
• Convergence Insufficiency (CI): This common condition causes overlapping or drifting text when reading. It frequently affects school-aged children as well as adults later in life. Because CI often causes recurring headaches, neurologists regularly refer patients for optometric evaluations to rule it out.
• Neurological & Traumatic Events: Head trauma, concussions, strokes, cranial nerve palsies, and conditions such as Multiple Sclerosis or Parkinson’s disease frequently impact eye muscle control.
• Age-Related Changes: In some cases, a mild, asymptomatic misalignment present in youth gradually progresses over time, eventually requiring correction.
